Job Overview
Position Title: Case Management Nurse

Location: Scottsdale, AZ

Hours & Benefits
Hours: Monday - Friday, 8:00 AM - 5:00 PM

Benefits: Medical/Dental/Vision/Life

Vacation: 80 hours per year – 1st year prorated

AZ Sick Time: 40 hours per year

Responsibilities

Completes an initial, comprehensive and ongoing assessment of patient and family to determine hospice needs. Provides complete physical assessment and history of current and previous illness(es).

Provides professional nursing care by utilizing all elements of nursing process.

Assesses and evaluates patient’s status by writing and initiating plan of care.

Regularly re-evaluates patient and family/caregiver needs.

Participates in revising the plan of care as necessary.

Initiates the plan of care and makes necessary revisions as patient status and needs change.

Uses health assessment data to determine nursing diagnosis.

Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions, including patient and family in planning.

Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by physician.

Counsels the patient and family in meeting nursing and related needs.

Provides health care instructions to the patient as appropriate per assessment and plan.

Assists the patient with activities of daily living and facilitates self-sufficiency and optional comfort care.

Acts as Case Manager when assigned by Clinical Supervisor and coordinates patient care for assigned caseload.

Qualifications

Bachelor’s degree from an accredited school of nursing.

One to two years of recent acute care experience in an institutional setting, and one to two years of recent experience in home care, plus one year of hospice care experience preferred.

Current licensure in State and CPR certification.

Responsible for supervising hospice aids.

Excellent observation, verbal and written communication skills, problem solving skills, basic math skills.

Must be able to walk and stand for long periods of time; able to lift, position and/or transfer patients; capable of reaching, stooping, bending, kneeling, crouching. Adequate vision and hearing.

Personal car for travel and valid driver’s license.

Carry personal auto liability insurance coverage.

Notes
This position requires a willing and able driver to travel to patients’ residences.
