Overview
We are looking for a high‑energy, compassionate, detail‑oriented Registered Nurse (RN) to be a Hospice Case Manager for patients in and around the Albuquerque area. Monday through Friday 8a‑5p with some on‑call requirements. We are committed to providing the best care possible.

Responsibilities
Respond to clinical referral information with care and expertise, addressing inquiries and requests.
Partner with the Clinical Director to assess eligibility and suitability of clients for hospice services.
Coordinate care, ensuring comfort and quality of life remain paramount.
Deliver top‑notch nursing care, easing pain and providing solace.
Educate and advocate for patients and families, encouraging participation in personalized care plans.
Share call duties as required.
Qualifications
Current unencumbered RN license.
Minimum one year experience as an RN in medical surgical/acute care; hospice experience a plus.
Computer proficiency in typing and various programs, EMR background.
Current CPR certification if required by state.
Compensation & Benefits
Pay range: $90,000 – $97,000 per year. Final compensation based on experience, qualifications, and responsibilities.
