Overview RN Case Manager (Corrections)
Competitive Pay of $3500 to $3600 weekly
ATC Northwest is looking for an RN Case Manager (Corrections) to work in Salem Corrections . The RN Case Manager provides comprehensive nursing case management services for incarcerated individuals within the corrections facility. This role ensures that patients receive timely, coordinated, and evidence-based healthcare while navigating the unique medical, mental health, and social service needs of the correctional population. The Case Manager RN works collaboratively with physicians, behavioral health professionals, custody staff, and external agencies to promote optimal health outcomes, continuity of care, and compliance with all applicable laws, regulations, and facility policies.

RN Case Manager (Corrections) Responsibilities

Assessment & Planning

Conduct initial and ongoing health assessments for inmates with acute, chronic, or complex medical conditions.

Develop individualized care plans addressing medical, behavioral, and psychosocial needs.

Identify and prioritize high-risk patients for intensive case management.

Coordination of Care

Facilitate communication among healthcare providers, custody staff, and outside specialists.

Arrange specialty referrals, diagnostic testing, and follow-up care as needed.

Coordinate discharge planning for inmates transitioning to community healthcare systems.

Patient Advocacy & Education

Advocate for appropriate and timely healthcare services within the correctional environment.

Provide patient education on chronic disease management, medications, and preventive health.

Promote self-care skills and compliance with prescribed treatment regimens.

Documentation & Compliance

Maintain accurate, complete, and timely medical records in compliance with facility, state, and federal standards.

Monitor and evaluate clinical outcomes, utilization of services, and adherence to case management protocols.

Support quality improvement initiatives and compliance with NCCHC/ACA standards.

Collaboration

Work closely with mental health and substance abuse treatment teams to provide integrated care.

Participate in interdisciplinary care meetings and case reviews.

Serve as a liaison with community healthcare resources and agencies for continuity of care upon release.

Qualifications

Current and unrestricted Registered Nurse (RN) license in the state of practice.

Minimum 2–3 years of clinical nursing experience; prior correctional healthcare, case management, or community/public health nursing preferred.

Certification in Case Management (CCM or ACM) desirable.

Strong knowledge of chronic disease management, discharge planning, and patient education.

Excellent assessment, critical thinking, and problem-solving skills.

Ability to work effectively within a correctional environment and adapt to security protocols.

Strong interpersonal and communication skills; ability to build rapport with diverse populations.
