RN Case Manager
Reports To: Clinical Team Leader
Status: Exempt (Salaried)

Shift: 40 hours; Monday - Friday 8AM - 4:30PM with rotating holidays per business needs
Location: Cherokee/Clay Counties, NC
Salary Range: $30.27 - $44.00 per hour
Position Summary
Coordinates the nursing plan of care, provides direct nursing care, and instructs/supervises the LPN and Hospice Aide staff in the delivery of nursing care to clients.
Manages the nursing care for an assigned caseload of approximately 15 patients with varying acuity and needs. Caseloads above this target will be co-managed or additional support will be offered. If the caseload falls below target, the nurse may be asked to provide support to other team members.
Essential Duties & Responsibilities Practice within the scope of license, certification or registration, where applicable, and as required by law and regulation
Assess the patient’s physical and mental health including the patient’s reaction to illnesses and treatment regimens
Record and report the results of the nursing assessment
Plan, initiate, deliver, and evaluate appropriate nursing acts
Teach, assign, or supervise other personnel in implementing the treatment regimen
Collaborate with other health care providers to determine the patient and family-centered plan
Implement the treatment and pharmaceutical regimen prescribed by any person authorized by State law to prescribe
Provide teaching and counseling about the patient’s health
Report and record the plan for care, nursing care given, and the patient’s response
Supervise, teach, and evaluate those who perform nursing functions and administer nursing services
Provide for the maintenance of safe and effective nursing care, whether rendered directly or indirectly
Knowledge, Skills & Abilities Willingness to care for patients with infectious disease processes unless accommodations are approved
Knowledge of Federal, State and Local regulations and Medicare Conditions of Participation
Knowledge of HIPAA privacy rules and regulations
Knowledge of electronic medical records systems
Demonstrate basic data entry skills
Demonstrate excellent organization, interpersonal, written and oral communication skills
Ability to work under pressure to meet deadlines
Ability to take initiative and utilize innovative techniques and ingenuity
Ability to interact with various departments and work flexible hours as needed
Ability to participate as a team player and coordinate multiple projects
Demonstrate skill/expertise in pain and symptom management
Demonstrate ability to collaborate and facilitate group work
Minimum Qualifications Experience: Two years of clinical experience preferred
Licensures & Certifications Valid NC RN license
Valid Driver’s License and proof of automobile insurance

Flexion/Extension of arms/wrist
Hand dexterity to include gripping, pushing/pulling and writing
Lifting/carrying of 50 lbs. often
Other Duties Work to achieve performance goals and participate in department and staff meetings
Ongoing education and training as required by nursing supervisor
Compliance with company policies and Mission Statement
Practice safety and infection control methods
Assist in providing 24 hour on-call services, including coverage of at least one holiday per year
Assess for the need of other disciplines
Perform other duties as assigned
