RN Case Manager – Charlotte, NC (Hybrid Remote)
DaVita Kidney Care is looking for a Full‑Time RN Case Manager to join our Charlotte, NC region. This role supports patients with complex chronic kidney disease (CKD) or end‑stage kidney disease (ESKD) by coordinating care, addressing medical, social, emotional, and financial needs, and ensuring quality, cost‑effective care across the care continuum.
Base Pay Range
$68,400.00 – $100,400.00 per year
Position Details
Location: Hybrid – work from home with travel within an assigned geography for in‑person visits (dialysis clinics, nephrology practices).
Schedule: Monday–Friday, flexible to accommodate patient and nephrology partner availability.
Full‑time position.
Rapidly build empathetic relationships with patients and families.
Coordinate care transitions, manage complex/at‑risk patients, and establish treatment plans in partnership with the care team.
Address social determinants of health including medical, emotional, educational, community, and financial concerns.
Ensure streamlined, effective transitions of care when needed.
Identify and assess unreported medical conditions or changes that may lead to adverse outcomes.
Requirements
Current RN license (BSN preferred).
Minimum 2 years RN experience in a healthcare setting, preferably with performance coaching.
CPR or BLS certification.
Valid driver’s license and willingness to travel within assigned territory.
Intermediate computer skills; proficient in MS Word, Excel, and Outlook.
Home office with at least 1 MB upload and 1 MB download internet speed.
Preferred Qualifications
2+ years of case management or chronic care management experience.
1+ year of utilization management experience.
Spanish speaking preferred for some markets.
