About the Job
The Case Manager RN will work primarily in the Emergency Department, and collaborate with other members of the health care team to identify appropriate utilization of resources and a safe and effective transition plan ensuring clinical and social determinants of health are met. With the patient, family/caregiver, and health care team, create a transition plan appropriate to the patient’s needs and resources including community providers to ensure effective communication and collaboration with a successful transition plan.
Schedule
You will work 36 hours per week, (3 12-hour shifts), and every other Saturday. Rotating every 6-8 weeks to cover both Sat/Sun with a “comp day” during the week before and week after to offset the weekend day.
What You’ll Do
Assesses and identifies patients’ needs during the discharge planning process
Provide resources and community referrals for continuity of care
Care coordination with families, providers, and clinical team
Follows Medicare conditions when needed
Utilization Reviews
Who You Are
Bachelor's degree in a healthcare-related field or equivalent experience
Current BSN or commitment to achieving the degree in a specified amount of time
3 – 5 years of case management-related health care experience.
CCM or ACM preferred
Excellent clinical documentation, and successful usage of EMR systems preferred
