Role focuses on in-home admissions, in-home supervisory visits, office duties, and semi-annual or annual on-site assessments of field caregivers. The team ensures compliance with client assessment of need and service plan standards in line with state regulations. Office hours are typically Monday Friday 9:00 AM 5:00 PM, with variation to support business needs.
Responsibilities Conduct initial evaluation visits and regularly re-evaluate clients' nursing needs.
Counsel clients and their families regarding nursing needs and related concerns.
Coordinate and create service plans following client assessments and admissions.
Assist and supervise field staff, ensuring successful implementation of new Medicaid Waiver programs.
Ensure quality care by creating, implementing, monitoring, and enforcing quality assurance policies and procedures.
Assist branch teams with pre-service training and orientation, as well as in-service training for caregivers.
Assist with other duties as needed and as assigned by the Branch Manager.
Qualifications A current RN license in the state of service
A valid driver's license
Access to reliable and insured transportation
Ability to travel within surrounding counties 2-3 days per week between 9:00 AM 5:00 PM
Availability to be on-call for caseload-related crises
Nursing experience preferred
