Overview St. Mary Home Care has a full-time opening for a Home Health Registered Nurse (RN) Case Manager serving Langhorne, Newtown and the greater Bucks area. This role supports one-to-one, compassionate care in patients\' homes.
Responsibilities Provide one-to-one, compassionate care and develop personalized care plans for Home Care patients.
Administer medications and support recovery and independence in patients
Utilize the Epic EMR system for streamlined documentation and communication
Deliver care in patients\' homes, maintaining a patient-centered approach
Qualifications Graduate of an approved nursing education program
Licensure as a Registered Nurse in the state of PA
One (1) year experience as a professional nurse; home care experience preferred
Must have current Driver\'s license and reliable transportation
Ability to work a weekend rotation once monthly and 2 holidays annually
