Job Type: Full-time (32+ hours/week)
Pay: $36-$42/hour DOE + $5,000 sign-on bonus (full-time only) + mileage reimbursement

We are seeking a compassionate and skilled Registered Nurse Case Manager to support PASCO clients in the Denver Metro area. In this home health role, you will oversee the comprehensive coordination of care and deliver skilled nursing visits to a dedicated caseload. This is an ideal opportunity for a nurse looking to gradually grow from part-time to full-time within a flexible, supportive environment.
KEY RESPONSIBILITIES Conduct home visits every 60 days (or as needed) to reassess care needs for CNA and RN services
Deliver skilled nursing care within RN scope of practice
Complete orders and resumption of care assessments within 48 hours of client hospital discharge
Educate clients and caregivers, providing guidance in Spanish on diagnosis, symptoms, and home care
Coordinate with interdisciplinary teams and ensure alignment of care plans and physician orders
Provide required CNA competencies and training
Maintain professional communication with providers, clients, and families
REQUIREMENTS Active and unencumbered Colorado RN license with 1 year nursing experience required
Current CPR/BLS card
Valid driver’s license and reliable transportation
Previous home health experience highly preferred
Excellent communication and time-management skills
Experience working in the disabled community highly preferred
