Overview
The Case Manager Registered Nurse (RN) provides comprehensive and coordinated care to patients. Working under moderate or general supervision, this role analyzes patient needs, participates in care coordination, conducts research, reconciles information, and reviews care plans.
Responsibilities
Analyze patient cases to assess needs and develop individualized care plans.
Prepare and process documentation related to patient care to ensure accuracy and compliance with regulatory requirements.
Research and stay up-to-date with evidence-based practices and healthcare guidelines relevant to the assigned patient population to incorporate the latest knowledge into care plans and interventions.
Review and evaluate the effectiveness of care plans, monitor patient outcomes, and make adjustments as needed to optimize patient well-being.
Experience: Required: 3 years as a Registered Nurse and 1 year in a clinical specialty as defined by ANCC or national nursing organizations.
Licensure/Certification: Required: Registered Nurse license in North Carolina or a Compact state; BLS (CPR) – American Red Cross or AHA Healthcare Provider.
