Transitions At Home is currently seeking to hire a full-time RN Case Manager. We’re looking for a compassionate and organized RN Case Manager to join our Home Health team. Base pay range
$34.00/hr - $38.00/hr
Transitions At Home is a non-profit home health organization serving Northern, Central, Western and Southern Wisconsin. The RN Case Manager provides skilled nursing care including assessments, medications, treatments, and coordinates services. The RN Case Manager counsels the patient and family in their place of residence in accordance with the patient’s physician orders and plan of care under the direction and supervision of the RN Clinical Manager.
Hours
Monday through Friday 8:00 am – 4:30 pm
No Night Shifts
Limited on-call rotation

Qualifications / Job Requirements
Active Wisconsin Registered Nurse License
Two (2) years’ nursing experience; 1 year in Home Health (preferred)
CPR Certification
A valid Wisconsin Driver's License and car insurance
