Overview Good Shepherd Hospice is seeking a compassionate candidate for a Registered Nurse Case Manager serving patients and their families in Tulsa and surrounding communities.
The RN provides nursing assessment, planning, and care to maximize the comfort and health of patients and families. The RN responds to all phone calls promptly and appropriately, makes patient visits as necessary, and completes after-hours admissions and deaths as needed.
Responsibilities Receives patient assignments from the Patient Care Manager or Case Manager. Initiates communication with attending physicians, hospice staff, and other agencies as needed to coordinate care and resources for the patient/family.
Cultivates communication and bereavement skills.
Maintains regular communication with the attending physician concerning patient/family care.
Contacts families of new patient assignments and maintains communication as requested by the family.
Gathers data on physical, psychological, social, and spiritual factors influencing patient/family health and incorporates this into the Plan of Care.
Maintains up-to-date patient records, clearly documenting problems, plans, actions, and goals, and reflects changes as they occur. Submits paperwork to the office per Medicare and hospice policies.
Instructs the patient and caregiver on:
Medication administration, recognizing side effects and adverse reactions.
Personal care and oral hygiene.
Assisting with lifting, moving, ambulation, and exercise.
Disaster and safety management.
Preparing for events preceding death.
Informs the Patient Care Manager of any unusual or potentially problematic patient/family issues.
Attends hospice team meetings and other patient conferences, including care plan meetings, as deemed necessary.
Provides appropriate support at the time of death.
Participates in providing 24/7 coverage to patients and families.
Evaluates hospice aides biweekly.
Evaluates LPNs at least once per month.
Engages in ongoing education to understand hospice philosophy.
Develops proficiency in pain management through reading, training, and seminars.
Performs all other assigned tasks.
