Registered Nurse (RN) Case Manager – Home Health Specialist
Company: Home Health Care, Inc.
Location: Southwest Minnesota Region
Schedule: Full‑time, Monday‑Friday 8:00‑5:00 AM, 1 weekend/month, 1 on‑call shift/week
Compensation: $85,000 – $130,000 annually + $8,000 in bonuses first year.

Responsibilities

Lead comprehensive nursing assessments and individualized care plans

Conduct OASIS visits and coordinate services across care settings

Collaborate with physicians and providers to maintain clinical continuity

Deliver education, emotional support, and transition planning for families

Ensure accurate documentation and regulatory compliance

Qualifications

3+ years of clinical RN experience; home health preferred

Strong documentation, care planning, and assessment skills

Reliable vehicle and valid driver’s license

MN RN license in good standing (required)
