Niagara Hospice is looking for a compassionate RN to join our home care team as a Case Manager.
Monday - Friday 8:30am - 4:30pm with a weekend required every 8 weeks and involvement in holiday rotation.

Essential Functions Case finding & Management: provide direct patient care and assessment, oversight of plan-of-care development and implementation.
Diagnosing: identification & discrimination between physical & psychosocial signs and symptoms essential to effective execution and management of the nursing regimen.
Treating: execution of prescribed medical regimen & direct orders from authorized healthcare providers, administer medications and treatments during home visits as needed to effectively control patient symptomatology.
Measuring Human Responses & Patient Outcomes: evaluate signs, symptoms and processes which denote individual interaction with an actual or potential health problem.
Health Teaching & Counseling: educate patients/families on palliative and end-of-life care and issues in a proactive manner, thoroughly explain procedures to patients before executing the same.
Care Supportive of Life and Well Being: demonstrate respect of patient/family choices, values and behaviors, advocates for the choices of patients/families.
Professional Communication & Documentation: communicates progress of patients to advanced practitioners accordingly, complete patient profiles/summaries promptly, record patient’s condition, treatment and response, chart required information in the MISYS Homecare System.
Indirect Patient Support & Management Activities: place referrals for necessary supportive services; act as a liaison to contracted hospitals and nursing facilities as assigned.
Professional Performance: demonstrate knowledge and competencies required of the position, understand & demonstrate knowledge of the NYS Nurse Practice Act, NYS and federal guidelines/codes governing Hospice and the accrediting body.
Education & Qualifications RN Licensure in New York State (required)
Graduate of an accredited school of professional nursing (required)
Minimum of three (3) years of nursing experience preferred
Valid NYS Drivers License
