Transitions At Home is currently seeking to hire a full‑time RN Case Manager. This position offers the opportunity to provide personalized care in a home setting, ensuring patients receive the highest quality of service and support throughout their recovery journey.
What does a Transitions at Home RN Case Manager do?
Registered Nurse Case Managers provide skilled nursing care including assessments, medications, treatments, and coordinate services. They counsel the patient and family/significant others in their place of residence in accordance with the patient’s physician orders and plan of care under the direction and supervision of the RN Clinical Manager.

40 hours per week 8:00 am - 4:30pm
Monday through Friday
Limited on‑call rotation

Qualifications / Job Requirements
Active Wisconsin registered nurse license
Two (2) years’ nursing experience, 1 year in home health (preferred)
CPR certification
Valid Wisconsin driver’s license and car insurance
