Base pay range $32.00/hr - $40.00/hr
Job responsibilities Case manage and provide skilled nursing services for patient visits in home health/hospice settings.
Coordinate home care services for patients, including supervision of CNAs and PRN nurses in the home.
Coordinate care between home care disciplines (PT, OT, ST, CNA, MSW).
Qualifications Must have graduated from an accredited school of nursing and hold a current RN license to practice in NC.
Experience: at least one year of experience as an RN in a hospital, home care, hospice, or public health setting.
Other: Valid NC driver's license and reliable transportation.
