Job Title: Registered Nurse (RN) Case Manager

Job Type: Full-Time (Monday to Friday 8:00am - 5:00pm) + 1 Weekend and on-call availability.

Compensation: $80,000 - $130,000 per year + $6000 in Bonuses During First Year of Employment! Ability to earn extra money with taking on more visits.

RN Case Manager Overview

As a valued member of our team, you'll:

Screen and assess clients, develop and implement patient care plans, and monitor care delivery to ensure safe, medically appropriate, and cost-effective care.
Complete OASIS timepoint visits, facilitate referrals, and coordinate care transitions between settings.
Communicate and collaborate with the patient's primary provider, healthcare and community providers, and other members of the interdisciplinary care team.
Provide patient/family education and counseling, assess psychological, psychosocial, and psycho-spiritual needs, and ensure compliance with organizational and regulatory standards.
Work with our multidisciplinary team to provide quality care tailored to each patient's physical and emotional needs.

Qualifications

To excel in this role, you'll need:

A current Registered Nurse (RN) license in the state of Minnesota, along with at least three years of nursing experience (home care experience preferred).
Knowledge of home care regulations, standards, and quality indicators, strong organizational, assessment, and problem-solving skills, and proficiency in computer systems and software applicable to job duties.
A valid Driver's License and reliable vehicle with current insurance.
