Transition At Home is hiring a full‑time RN Case Manager to serve the Sauk City area. This position coordinates and oversees patient care in the home setting, ensuring personalized, high‑quality outcomes for every client.
Responsibilities
Registered Nurse Case Managers provide skilled nursing care that includes assessments, medications, treatments, and coordinate services. They counsel the patient and family/significant others in their place of residence in accordance with the physician’s orders and plan of care under the direction and supervision of the RN Clinical Manager and Administrator.

Qualifications
Active Wisconsin Registered Nurse license
Two (2) years of nursing experience, 1 year in home health (preferred)
CPR certification
Valid Wisconsin driver’s license and car insurance
