Overview Transitions At Home is seeking a full-time RN Case Manager in the Dodgeville area. Our growing home health agency coordinates and oversees patient care in the home setting to provide personalized, high-quality outcomes. We offer a flexible work schedule and a supportive team culture.
Responsibilities Provide skilled nursing care to include assessments, medications, treatments, and coordination of services in the patients residence in accordance with physician orders and the plan of care, under the direction and supervision of the RN Clinical Manager and Administrator.
Counsel the patient and family/significant others as part of the care planning and ongoing management.
Hours 40 hours per week, 8:00 am 4:30 pm
Monday through Friday
No night shifts
Limited on-call rotation

Qualifications Active Wisconsin Registered Nurse License
Two (2) years nursing experience, 1 year in Home Health (preferred)
CPR Certification
A valid Wisconsin Driver's License and Car Insurance
