Join Avow, Inc. as an RN CASE MANAGER . We are expanding our services into Lee County and offer a $10,000 Sign On Bonus for qualified full-time RN roles. The schedule is Monday through Friday, 8:30am to 5:00pm, with no nights or weekends.
Job Summary: Under physician's orders, the RN Case Manager identifies the physical and emotional needs of the patient/family and coordinates the necessary care to meet these needs, maximizing patient comfort and enhancing the quality of life for the patient/family.
Job Duties And Responsibilities:
Assess the physical and emotional needs of the patient to provide a framework for patient care planning.
Complete the Patient Assessment Form/Patient Care Note within established guidelines and document findings in a timely manner.
Recognize, report, and document significant findings/changes in the patient's condition to the clinical manager and/or physician.
Assess the need for medical supplies, equipment, and medication on a regular basis.
Contribute to the development of an Interdisciplinary Treatment Plan in collaboration with the multidisciplinary team.
Identify additional problems/needs and update the plan of care as necessary.
Implement and direct the plan of care in an organized and efficient manner.
Communicate with other team members and outside services to provide coordination of care.
Provide information and direction to HHAs/CNAs, documenting supervision according to policy.
Complete documentation accurately, concisely, and in a timely manner.

Education/Experience: Bachelor's degree or equivalent combination of education and experience.
Language Skills: English communication skills; Spanish language skills preferred.
Computer Skills: Strong computer skills, including database software and email/Outlook.
Certificates, Licenses, Registrations: Current RN License to practice in Florida, valid Florida Driver's License, and Healthcare Provider CPR.
Physical Demands: The employee may be required to sit, stand, walk, and reach with hands and arms, use hands to finger, handle, or feel, and lift and/or move up to 50 pounds.
Work Environment: The employee may be exposed to outside weather conditions, and the noise level is usually moderate.
