Required Experience and Education Education: Graduate of an accredited Nursing program
Licenses and Certifications: Texas Nursing Licensure Compact (NLC) current license or NLC Nursing Licensure Compact license and Basic Life Support (BLS) approved by the American Heart Association (AHA) or American Red Cross (ARC)
Preferred Education: Bachelor\'s Degree in Nursing
Experience: 2 years as an RN Care Coordination / Case Management in an acute care hospital setting
Job Duties Leads multidisciplinary team developing, refining, updating, and communicating plan of care for defined patient population
Implements clinical pathways including patient/family education, monitoring, and updating for deviations from the plan
Partners with community liaisons to proactively manage care transitions and ensure seamless transitions between care settings with ongoing providers
Collaborates with Social Work, Utilization Review, Clinical Documentation Specialists, and others to maximize financial and care resources for patient and family
Coordinates care with the care team to maximize patient throughput and minimize length of stay
Rounds on patients daily to monitor and assure effective discharge preparation and completion of learning/discharge objectives prior to planned discharge
Communicates effectively with other health care providers, patients, families, and hospital personnel; demonstrates appropriate communication across age groups
Acts as a liaison with departments and ancillary services to ensure timely receipt of orders and results
Ensures follow-up appointments are scheduled prior to discharge and that discharge instructions related to care coordination are understood by patient and caregiver
Collaborates with physicians, unit manager, unit staff, and interdisciplinary team to ensure safe and timely discharge
Performs other duties as assigned
