Position Purpose / Summary
Provides skilled nursing assessments, planning, and care in compliance with the interdisciplinary team plan of care under the supervision of the Director of Palliative Care and the direction of the physician according to the policies and procedures of Hospice of San Joaquin and the Palliative Care program.

Essential Duties and Responsibilities

Provides palliative education, emotional support, and community resource information to program participants via telephone or home visits.

Completes the initial nursing assessment and identifies the physical, emotional, spiritual, physiological, and practical needs of the patient and family.

Develops treatment plans based on identified needs with measurable outcomes.

Utilizes Hospice of San Joaquin electronic medical record system for patient documentation and maintains up‑to‑date records in accordance with Medicare, Medicaid, State home health agency, and JCAHO standards.

Updates the plan of care and suggests actions to the IDT for follow‑up by various team members.

Interventions reflect an advanced level of nursing practice and commitment to the execution of the IDT plan of care.

Ensures HIPAA compliance.

Provides referral outcome information to the managing physician and educational visits to referral sources/agencies.

Maintains a professional appearance, exhibits respectful and courteous behavior with customers/participants and staff at all times.

Uses expert knowledge of hospice nursing, crisis intervention, and family dynamics to determine the appropriate level of care and advocate for patients.

Collaborates with pharmacy, DME, and other health care providers and IDT members to provide appropriate care.

Coordinates care with skilled nursing facilities or acute care hospitals when necessary and functions as an on‑call nurse.

Follows universal precautions, demonstrates creativity in meeting patient/family needs in the home environment, and supervises and documents the Home Health Aide plan of care.

Additional duties assigned by the Director of Palliative Care.

Qualifications
Graduate of an accredited school of nursing in California as a Registered Nurse. Three years of recent clinical experience, computer skills, and ability to learn and assimilate the clinical computer program system. Knowledge of advancing chronic or life‑limiting illness/conditions and ability to handle multiple projects simultaneously. Excellent verbal and written communication skills. Ability to write clear, concise reports. Superior problem‑solving and critical‑thinking abilities. Reliable transportation and a valid California driver’s license. All employees who drive must provide evidence of insurance.

Physical Demands
Regular requirement to talk, hear, sit, stand, bend at the waist, walk, climb stairs, reach with hands and arms, drive distances including freeways and mountains when necessary. Reasonable accommodations available.
