Overview We are seeking a passionate and experienced RN Case Manager to join our dynamic healthcare team. This full-time, permanent position offers an exciting opportunity to work in a supportive and collaborative environment, providing the highest quality care to patients in need. The RN Case Manager will oversee the coordination of patient care from admission to discharge, ensuring care is effective and efficient. The ideal candidate will have a strong background in Pain Management, Palliative Care, Home Care, and Geriatrics and will be dedicated to improving patient outcomes while reducing healthcare costs.
Responsibilities Coordinating and managing the care of patients to ensure health needs are met and care is provided in the most effective and efficient manner.
Developing, implementing, and updating individualized care plans based on patient assessments and input from the healthcare team.
Providing pain management, palliative care, and home care as needed, and offering expert guidance on geriatric care.
Advocating for patients and serving as a liaison between patients, families, and the healthcare team.
Collaborating with physicians, nurses, and other healthcare professionals to ensure a seamless continuum of care.
Monitoring patient progress and adjusting care plans as necessary.
Educating patients and their families about their conditions and steps to improve health.
Participating in quality improvement initiatives and contributing to the development of best practices in patient care.
Qualifications Bachelor’s degree in Nursing. Master’s degree in Nursing or related field is preferred.
Current and valid RN license.
A minimum of 1 year of nursing experience, with a focus on Pain Management, Palliative Care, Home Care, and Geriatrics.
Excellent case management skills with the ability to coordinate and manage patient care effectively and efficiently.
Strong communication and interpersonal skills and the ability to collaborate with a diverse healthcare team and build strong relationships with patients and families.
A deep understanding of healthcare systems, medical terminology, and healthcare legislation.
The ability to work independently and make critical decisions related to patient care.
A commitment to continuous learning and professional development.
Base pay range $100,000.00/yr - $130,000.00/yr
