The Registered Nurse Case Manager provides a wide range of nursing care to patients in the home care setting. The RN Case Manager renders professional nursing care by assessing, developing, implementing and evaluating home care needs.
Responsibilities
Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients.
Assess, plan, direct and evaluate each patient’s care plan to ensure it meets the patient's needs.
Manage emergency situations in a prompt, precise, and professional manner.
Complete medication administration and treatment needs for all patients assigned.
Provide nursing care in accordance with applicable regulations and company policies.
Prepare and maintain accurate and complete clinical and progress notes on a timely basis.
Provide supervision and oversight to patient care.
Utilize the Electronic Visit Verification system.
Prepare physician orders as applicable.
Participate in continued education programs, patient care conferences and staff meetings as scheduled.
Perform nursing assessments (initial and ongoing).
Maintain excellent assessment skills and clinical knowledge and expertise.
Notify the physician and branch management of significant changes in patient status and obtain changes in orders as necessary.
Assume responsibility for ongoing interdisciplinary assessment of the patient.
Participate in branch on-call rotation as assigned.
Follow organizational policies and procedures regarding infection control, disposal of hazardous waste and use of protective devices as evidenced by documentation.
Perform other job-related tasks and duties as assigned.
Ability to work flexible hours as required to meet identified patients’ needs.
Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the patient.
Able to work independently.
Must possess a strong commitment to the goals, mission, and philosophy of the organization.
Understands the concepts of home health and the role of the RN as a member of a full discipline health care team.
