Job Summary
This is a fully remote Nurse Case Manager role that requires home visits and occasional visits to nursing facilities. The case manager will assess, plan, implement, and coordinate care for patients within a 50‑75% travel territory that includes Dallas, Ellis, Navarro, Kaufman, Rockwall, Hunt, Collin, and Tarrant counties.
Responsibilities
Telephonically and/or face‑to‑face assessment of members’ medical needs to facilitate overall wellness.
Develop proactive courses of action to address short‑ and long‑term outcomes.
Coordinate and implement assigned care‑plan activities and monitor progress.
Conduct multidisciplinary reviews to achieve optimal outcomes.
Identify and accelerate quality‑of‑care issues through established channels.
Utilize negotiation, motivational interviewing, and influencing skills to secure appropriate services and promote lifestyle/behavior changes.
Provide coaching, information, and support to empower members to make ongoing independent medical and healthy lifestyle choices.
Help members actively participate with providers in healthcare decision‑making.
Document care through monitoring, evaluation, and quality‑management processes in compliance with regulatory and accreditation guidelines.
Qualifications
Registered Nurse with an unrestricted active license in Texas.
Current unrestricted state licensure and up‑to‑date credentials.
3+ years of clinical practice experience.
1+ year of case‑management experience.
Comfortable driving and able to travel up to 75% of time within the specified territory.
Position Information
Full‑time employment with a salary of $38–$39 per hour. No bonus or commission.
