An Opportunity to Join our Remarkable Care Team as a Case Manager in the Case Management Department awaits YOU!!! Trinity Health Livonia is a beautiful full‑service 304‑bed acute care hospital located in Livonia, MI, that provides comprehensive care including a 24‑hour emergency department, general medicine, inpatient and outpatient surgery, physical medicine and rehabilitation, intensive care unit, cancer, cardiology, geriatrics and birthing and women's health. Through several major expansions in the hospital, programs and services, Trinity Health Livonia continues to offer the latest in quality health and medical services. Trinity Health Livonia has received numerous awards recognizing excellence in clinical outcomes, patient safety, financial performance and efficiency. A Case Manager is required to function at an advanced level of nursing and is required to understand the importance of disease management, critical pathways, continuum of care from admission through to discharge. The Case Manager will assess patient needs, facilitate inpatient planning using criteria sets, collaborate with physicians and other healthcare providers to develop a safe and appropriate plan of care for patients. The Case Manager may work in the ED, the Birthing Center or on the medical/surgical floors with either inpatients or Observation patients depending on their pre‑determined assignment.

Requirements

Minimum Education: Bachelor of Science degree in Nursing or health‑related field, or equivalent work experience.

Licensure / Certification: Current licensure to practice nursing in Michigan; progression toward or a completed certification in case management required; Current Basic Life Support (BLS) certification.

Clinical Experience: Five years of nursing experience and recent (within 2 years) experience in utilization review/management/discharge planning or case management.

Knowledge of third‑party payor programs and requirements; knowledge of federal, state and local regulations affecting utilization review programs and payments; knowledge of regulatory and accrediting agency standards and regulations relative to utilization review.
