We are looking for a high-energy, compassionate, detail-oriented Registered Nurse (RN) to be a Hospice Case Manager for our patients. Our hospice RN case managers plan, organize, and direct hospice care utilizing the nursing process of assessment, planning, interventions, implementation, and evaluation; they effectively interact with patients, caregivers, families, and interdisciplinary team members while maintaining professional nursing standards and clinical competency.

Responsibilities
Respond to clinical referral information with expertise and compassionate service.
Partner with our Clinical Director to assess the eligibility and suitability of clients for hospice services.
Coordinate care, ensuring patient comfort and quality of life remain paramount.
Deliver top‑notch nursing care, easing pain and providing solace to those under our care.
Educate and empower patients and families, encouraging active participation in personalized care plans.
Share call duties, demonstrating commitment to being present when needed.
Qualifications
Current, unencumbered RN license.
Minimum 1 year of experience as an RN in a medical‑surgical or acute care setting (hospice experience a plus).
Computer proficiency, including background in EMR and typing.
Current CPR certification required by state, if applicable.
Compensation
$98,000 - $108,000 per year (Average Pay Range) . Final compensation will be based on relevant experience, qualifications, and specific responsibilities of the position and will be confirmed during the interview process.
