Responsibilities
Deliver high‑quality nursing care in home health settings using the nursing process.
Assess patient needs and collaborate on individualized care plans.
Educate patients and families using population‑relevant materials.
Evaluate care outcomes and adjust plans as needed.
Coordinate with interdisciplinary teams to ensure continuity of care.
Participate in performance improvement and cost‑effective care initiatives.
Stay current with research and apply evidence‑based practices.
Skills & Qualifications
Active RN license in North Carolina.
At least 1 year of experience in Med‑Surg, ED, ICU, or similar acute care.
Strong clinical judgment and ability to manage complex patient needs.
Excellent communication and collaboration skills.
