Job Summary
University of Arkansas for Medical Sciences (UAMS) seeks an RN Care Manager to oversee patient care from admission through discharge. The position requires an understanding of UAMS policies, CMS, EMTALA, and other regulatory requirements. Responsibilities include coordinating interdisciplinary care, ensuring compliance, and facilitating discharge planning.

Responsibilities

Initiate appropriate referrals to internal and external interdisciplinary teams.

Establish the working diagnosis at initial review.

Communicate target length of stay and coordinate discharge planning with the interdisciplinary team.

Document all team, physician, and patient/family communication regarding coordination of care.

Coordinate with physicians regarding admission status when criteria are not met.

Identify and intervene to ensure timely discharges and document avoidable or denied days.

Identify psychosocial/non‑medical discharge needs and refer to Social Work as appropriate.

Assess and revise discharge plans based on patient appropriateness.

Refer cases for Ethics Committee consideration when needed.

Participate in performance improvement and evidence‑based practice/research.

Support evidence integration into nursing practice.

Promote a positive educational environment for students and staff.

Provide patient/family education and implement POC using interdisciplinary, patient and family centered care.

Communicate effectively with multidisciplinary team and document in the medical record.

Report variances in care, systems, or processes through appropriate channels.

Act as a resource and demonstrate commitment to professional development of other UAMS employees.

Participate in staff recruitment/retention and develop evidence‑based nursing standards.

Perform other duties as assigned.

Qualifications

Minimum Qualifications:

Baccalaureate Degree in Nursing, ADN, or Diploma RN Degree

Three (3) years RN experience with current BLS certification

Valid Arkansas Registered Nursing License

Preferred Qualifications:

National Nursing Certification

BSN preferred; Certified Case Manager (CCM) or American Case Management (ACM) certification

Additional Responsibilities/Competencies:

Salary: Commensurate with education and experience.
