Job Summary
The Emergency Room/Department Care Manager will assess all potential admissions for meeting criteria for an acute care admissions and into the proper status based on MCG or 3rd party criteria. ER CM will assess avoidance of social admissions, and support at risk patients with community services to meet needs for health care and daily living needs. Will secure choice if patient may go to a nursing home or go into a hospice status. Verify patient’s next of kin phone number is correct on the ED record. Be an informational resource for the ED Providers.

Essential Functions

Provides ED patient assessments prior to admission.

Supports proper placement of patient in the appropriate status based on MCG or 3rd party review tool.

Avoids social admissions by supporting community resources and guidance.

Supports patient and family with admission support and discharge guidance with choice selection of POC known at time of admission.

Verifies next of kin's name and phone number is correct and on the patient's demographic sheet.

Performs other duties as assigned.

Complies with all policies and standards.

Qualifications

Associate Degree Registered Nurse required

Bachelor's Degree BSN with ADN preferred

1-3 years Acute Care / Hospital setting required

1-2 years Care Management Experience preferred

Licenses and Certifications

RN - Registered Nurse - State Licensure and/or Compact State Licensure Nursing license required
