Work Hours 8:00 am – 4:30 pm, Monday – Friday (Full‑time, Hourly)
Responsibilities Provide high‑quality nursing care in centers, clinics, contracted facilities, and participants’ homes.
Assess, plan, and coordinate participants’ home‑care services and develop interventions as part of the interdisciplinary team (IDT).
Participate in the 24/7 on‑call triage process for participant needs.
Monitor acute and chronic care needs across all settings and coordinate direct care and transitions.
Reconcile facility medication administration records, communicate discrepancies, and follow up on specialist visits.
Teach participants, caregivers, and families about self‑care, medication management, infection control, and safety.
Participate in all IDT meetings, utilization review meetings, and quality improvement projects.
Delegate tasks to CNAs and home‑care aides and evaluate their competence.
Perform other duties as required by the PACE Program policies and OSHA safety guidelines.
Qualifications Bachelor of Science in Nursing preferred; RN license required (NJ: New Jersey State Board of Nursing).
BLS certification within 90 days of employment.
One year of experience caring for frail or elderly populations; training provided if needed.
Experience in home care, long‑term care, or managed care preferred.
1 year of RN direct‑care experience required.
Reliable transportation, valid driver’s license, and auto insurance.
Medically cleared for communicable diseases and up‑to‑date immunizations.
