Overview
RN Care Manager, (Full time weekdays) – University of Arkansas for Medical Sciences (UAMS).
Open to eligible applicants; this position requires RN licensure and applies a continuum-of-care approach from admission through discharge.
Responsibilities
Initiates appropriate referrals to internal and external interdisciplinary teams.
Establishes a working diagnosis at the initial review and coordinates discharge planning with the interdisciplinary team.
Communicates target length of stay and discharge plan; documents all team, physician, and patient/family communications related to care coordination.
Communicates with physicians regarding level of care or admission status when criteria are not met.
Identifies and addresses discharge delays and avoidable/denied days; refers psychosocial and non-medical discharge planning needs to Social Work as appropriate.
Assesses patients prior to discharge and revises plans as needed.
Identifies cases for Ethics Committee consideration when appropriate.
Participates in Performance Improvement and Evidence-Based Practice processes; supports integration of current evidence into nursing practices.
Promotes a positive educational environment for students and staff; provides patient/family education.
Implements the care plan using an interdisciplinary approach with Patient and Family Centered Care; collaborates with the multidisciplinary team at all levels.
Performs additional duties as assigned.
Qualifications
Minimum Qualifications:
Baccalaureate Degree in Nursing, ADN, or Diploma RN Degree
Three (3) years RN experience with current BLS certification
Valid Arkansas Registered Nursing License
Preferred Qualifications:
National Nursing Certification
BSN preferred; Certified Case Manager (CCM) or American Case Management (ACM) certification
