Join Arkos Health, a value-based healthcare company delivering in-home, virtual, and in-clinical care. Location: field-based travel to patients’ homes covering the Atlanta, GA region (Cumming, Marietta, Suwanee, Dahlonega, Dawsonville, Braselton, East Cobb, Sandy Springs, Gainesville GA). Work Monday‑Friday, day shifts only, no major holidays or on‑call.
Key Responsibilities
Work directly with the Arkos Health Interdisciplinary Team.
Implement and update patients’ plans of care; coordinate comprehensive care and obtain referrals to interdisciplinary services (Home Health, Palliative Care, Social Services).
Meet patients in their homes at a minimum every 2 weeks for Intensive Case Management; increase visits as needed.
Complete Clinical and Social Assessments to develop care plans upon initial start of care visit.
Provide communication with patient’s providers and medical specialists regarding status changes.
Engage with Arkos Health leadership and contracted providers to resolve status changes, complaints, or complications via weekly ICT meetings.
Complete medical reconciliation for all transitional care patients within 30 days of initiation of TCM contact.
Qualifications & Skills
Registered Nurse license and graduation from an accredited RN program.
BLS/CPR certification.
Annual TB test negative; Hep B vax/titer proof.
Current flu shot (or waiver).
In‑depth knowledge of health care services and medical terminology.
Excellent communication, critical thinking, and time management skills.
Experience developing and revising care plans based on patient needs.
Experience working in a managed‑care organization in a field‑based or hybrid environment.
Strong technology skills and experience with electronic documentation tools.
Experience with disease‑management programs a plus.
