The RN Care Manager performs the initial assessment of patients to determine the level of care required. This role involves collaborating with healthcare providers to develop and implement care plans, ensuring that patients receive the appropriate level of care and support. The RN Care Manager works in a full-time capacity within the Care Management department, primarily during the day from Monday to Friday, with a weekend rotation on Saturday and Sunday from 9 AM to 5 PM.
