Responsibilities Reporting to the Supervisor of Care Management, this position provides case management to our members, including addressing medical, social, psychological, physical, and spiritual needs. Responsibilities include developing, implementing, and monitoring care plans in conjunction with the PCP, caregivers, and other team members to meet the member's needs.

Qualifications Associate’s Degree in Nursing for experienced candidates

NYS RN license required

Salary Range USD $90,000.00/Yr. - USD $110,000.00/Yr.
