Role Overview RN Care Manager Strive Health Services LLC

As an RN Care Manager, youll support patients with chronic kidney disease (CKD) and end?stage renal disease (ESRD), working closely with local providers and dialysis facilities to improve outcomes and reduce emergency dialysis.

What Youll Do Strives RN Care Managers are a critical part of supporting patient care for individuals with CKD and ESRD. You will coordinate services to optimize care delivery through telecommunication and in?home visits, serving as a vital resource for patients, families, local care teams, and healthcare professionals.
Responsibilities Work closely with the care team to craft personalized and thorough care plans addressing the clinical and non?clinical needs of each patient.
Conduct regular assessments onsite and virtually to detect undisclosed medical or social issues and changes in the home environment that could pose risks.
Initiate proactive outreach to patients and/or clinics as outlined in program guidelines to evaluate status, offer assistance, and address inquiries promptly.
Manage a diverse patient panel, swiftly and accurately evaluating individual needs while adhering to NCQA standards for complex case management.
Communicate updates to the care team and family members, identifying gaps in patient understanding and delivering tailored education and resources.
Act as a liaison with dialysis facilities, conveying care plan updates to facilitate smooth transitions in care, and maintain accurate program software records.
Generate and evaluate reports using Strive data platforms to identify trends, anomalies, and areas requiring focus.
Provide culturally sensitive care to meet diverse needs of Strive patients, fostering collaboration among all clinical team members.
Utilize clinical judgment, analytical skills, regular attendance, and comprehensive knowledge of case management to deliver high?quality care.
Provide in?person patient care, including standing, sitting, walking, pushing, pulling, and lifting.
Minimum Qualifications Associates degree from an accredited school of nursing.
Current RN license in the state of practice.
Three or more years of related education, experience, or certification.
Current BLS or CPR certification.
Efficient and reliable transportation, including an active drivers license, to travel across an assigned region.
Minimum Internet connectivity of 3.8?Mbps download/3.0?Mbps upload.
Preferred Qualifications Three or more years of experience in nephrology, ER, ICU, cardiac, or ambulatory complex case management nursing.
Certified Case Manager (CCM) certification preferred.
Proficiency in Microsoft 365 Suite (Word, Outlook, Excel).
About You Experience developing strong patient and family relationships that foster engagement and best outcomes for all aspects of Strive Healths Model of Care.
Demonstrated experience with proactively contacting patients and/or clinics frequently to assess both clinical and non?clinical needs.
Proficiency in documenting and navigating electronic medical records (EMR) and care plan management.
Salary Annual salary range: $74,000 $90,000
