Aging Life Care Professionals

Advocate for older and/or disabled adults and their loved ones to help make decisions related to healthcare, aging and long-term care planning which will:

  • Reduce hospitalizations
  • Improve quality of life
  • Decrease the burden on the family or responsible party

Recognize and advocate for the client's values and wishes, rather than your own.

Conduct, develop, monitor, and manage care plan and assessments to identify:

  • Issues and problems
  • Eligibility for assistance
  • Need for services

Onboard and sign up new clients, educating them on how our Care Managed Home Care (CMHC) services will help them age in place.

Review financial, legal and/or medical issues and offer referrals and recommendations to specialists and other professionals focusing on avoiding future problems and conserving assets.

Assist with determining the level of care needed and moving a client to a more appropriate facility or the arrangement of in-home care.

Act as liaison to responsible parties and professionals.

Facilitate our Care Managed Home Care model by:

  • Manage intakes, complete on-site safety assessments and complete care plans for new clients
  • Provide coaching for caregivers with new clients to ensure success
  • Overseeing the orientation and training to new and existing caregivers

Complete documentation requirements accurately and timely.

Participate in marketing efforts with the goal of educating the public, cultivating clients, and securing new clients.

  • Education and advocacy
  • Family caregiver coaching

Compensation: $30.00 - $35.00 per hour
