What You’ll Do
Strive’s RN Care Managers are critical to supporting patients with Chronic Kidney Disease (CKD) and End‑Stage Renal Disease (ESRD). They coordinate care delivery, primarily via telecommunication, with optional in‑home visits as needed.
Collaborate with the care team to craft personalized, thorough care plans.
Conduct regular assessments onsite and virtually to detect medical or social risks and refer appropriately.
Initiate proactive outreach to patients and/or clinics per program guidelines.
Manage a diverse patient panel, evaluating needs and adhering to NCQA standards for complex case management.
Communicate updates to the team and families, ensuring clear understanding and education.
Serve as liaison with dialysis facilities, conveying care plan updates.
Generate and evaluate reports using Strive data platforms to identify trends and focus areas.
Provide culturally sensitive care and collaborate with all clinical team members.
Utilize clinical judgment, analytical skills, and comprehensive case management knowledge to deliver high‑quality care.
Provide in‑person patient care that may require standing, sitting, walking, pushing, pulling, and lifting.
Minimum Qualifications
Associate’s degree from an accredited nursing school.
Current Registered Nurse (RN) license in the state of practice.
3+ years of combined related education, experience, or certification.
Current BLS or CPR certification.
Reliable internet connectivity (minimum 3.8 Mbps upload, 3.0 Mbps download, Valid driver’s license and reliable transportation for travel across the assigned region.
Preferred Qualifications
3+ years of experience in nephrology, ER, ICU, cardiac, or complex case‑management nursing.
Proficiency in Microsoft 365 (Word, Outlook, Excel) for a hybrid‑remote environment.

Annual Salary Range
$74,000.00 – $90,000.00
