A Day in the Life of a Respiratory Therapy Care Manager:
The Respiratory Therapy Care Manager (RTCM) performs assessments, provides education, and sets up home oxygen, BIPAP, CPAP and noninvasive ventilator set ups. The RTCM collaborates with social work, discharge planner, TACT nurses and multidisciplinary team to ensure patients have the appropriate oxygen resources both in and out of the hospital setting. Their focus is on quality patient care, optimal experience and readmission reduction.

Here Is What You Will Be Doing:
Performs assessments and screening of patients with COPD/respiratory diagnosis to anticipate home discharge needs.
Collaborates with inpatient and outpatient physicians, TACT nurses and Care Management ensuring patients have appropriate and timely respiratory needs at discharge and throughout the transition of care.
Coordinate setup with oxygen companies.
Sets up all home oxygen, BIPAP and NIV.
Ensures that patients who need to travel have appropriate equipment.
Provides COPD/other respiratory specific diagnosis education
Coordinates tobacco cessation, Pulmonary Function Test (PFT), and pulmonary rehab referrals.
Documents all patient communications and coordination in the medical record as appropriate.
Works with Safety net program to ensure appropriate use of program.
Desired Skills:
Excellent oral/written communication skills necessary to positively interact with patients, patient’s families, visitors, physicians and other staff
Ability to function independently and as a team member
Ability to assess, evaluate, explain, teach, problem solve using critical thinking; and perform calculations
Here Is What You Will Need:
Wyoming Respiratory Therapy License
Registered Respiratory Therapist (RRT) or Certified Respiratory Therapist (CRT)
Must have clean driving record that follows Cheyenne Regional medical Center’s driving guidelines
Cheyenne Regional AHA RQI within 14 calendar days of start date
6 Months : Tobacco Cessation Educator Certificate within 6 months of start date
Nice To Have:
Prior experience as a Respiratory Therapist
