This position is eligible for a [prorated] $1,000 sign-on bonus.
Job Summary: The Providence Center Residential Case Manager will provide clinical case management services to facilitate program goals and objectives using a quality care approach. Identify client needs with a person-centered approach to address mental health symptoms and co-occurring disorders, working with consumers to achieve the highest level of independence possible.
Schedule: Saturday first shift, 7:00am - 3:00pm; Sunday second shift, 3:00pm - 11:00pm
Duties and Responsibilities: Initiate treatment and advocate for consumers within the network of care.
Assist in developing and implementing socialization and recreational activities within the group home, in the community, and through Day Program groups and activities.
Complete recordkeeping in accordance with Health Information Services (HIS) and other Center requirements, maintaining confidentiality.
Provide case management and support to consumers.
Make assessments and provide crisis interventions and risk management as needed to ensure safety.
Teach consumers adult daily living skills to assist in all aspects of their life in the group home and supervised apartments.
Transport consumers in own vehicle or Center van to activities and appointments.
Maintain cooperative relationships with staff, consumers, community agencies, and the public.
Requirements: High School Diploma/GED and/or at least 5 years of experience in a behavioral health setting.
Experience in residential facilities preferred.
Valid driver's license, registration, current inspection, and insured automobile required as driving may be necessary.
