Job Summary:
The Providence Center Residential Case Manager Brick Manor PER DIEM provides clinical case management services to facilitate program goals and objectives using a quality care approach.
Identify client needs with a person-centered approach to address mental health symptoms and co-occurring disorders.
Work with consumers to achieve the highest level of independence possible.
Duties and Responsibilities:
Initiate treatment and advocate for consumers within the network of care.
Assist in developing and implementing socialization and recreational activities within the group home, in the community, and through Day Program groups and activities.
Complete recordkeeping in accordance with Health Information Services (HIS) and other Center requirements, adhering to confidentiality policies.
Provide case management and support to consumers.
Make assessments and provide crisis interventions and risk management as needed to maintain a safe environment.
Teach consumers adult daily living skills, assisting them in all aspects of life in the group home and supervised apartments.
Transport consumers using own vehicle or Center van to activities and appointments.
Maintain cooperative relationships with staff, consumers, community agencies, and the public.
Perform other duties as assigned.
Requirements:
High School Diploma/GED required; Associate's degree preferred.
Experience in residential facilities preferred.
Valid driver's license, registration, current vehicle inspection, and insurance are required.
