Residential Care Manager (532) (Healthcare)

Kings View is a nonprofit leader in providing behavioral health services to the underserved community. We are seeking a driven Residential Care Manager to join our mission to provide compassionate services to those with limited resources. Our ideal candidate will have a passion to promote social justice and societal awareness, respect for human dignity, and lead with integrity while listening and responding to the community in need. Location: Visalia, CA.

Responsibilities

Ensure the efficient operation and management of a residential care facility and provide comprehensive support to residents.

Interact directly with Members, their families, Authorized Representatives (ARs), caretakers, and other support persons.

Collaborate with a multi-disciplinary care team to address gaps in care and coordinate services across health domains, focusing on whole person wellness and recovery.

Engage with Members, families, ARs, caretakers, and other support persons to provide high-quality care and support.

Conduct outreach, education, coordination, health promotion, assessment, and care management.

Develop and implement care plans targeting the most vulnerable persons in the community.

Ensure the integration of services addressing social determinants of health (SDOH) and coordinate with healthcare and social service professionals to assist Members with funding, medical care, Veterans benefits, SSI, mental health, and other needs.

Advocate on behalf of Members with healthcare professionals; use motivational interviewing, trauma-informed care, and harm-reduction approaches.

Ensure a seamless experience for Members and non-duplication of services. Offer services where the Member lives, seeks care, or finds most accessible within Managed Care Plan (MCP) guidelines.

Connect Members to other social services and supports, including transportation; coordinate with hospital staff on discharge plans.

Accompany Members to office visits as needed and according to MCP guidelines.

Monitor treatment adherence, including medication; provide health promotion and self-management training.

Monitor vital signs if nursing certified; assist in the day-to-day operations of the residential care facility.

Ensure the facility is maintained in accordance with health and safety standards.

Oversee ECM services, including assessment, care plan development, and implementation; assess the needs of residents upon admission and throughout their stay.

Engage eligible Members to promote whole person wellness and recovery; focus on housing, symptom recovery, and avoidance of involvement with the criminal justice system.

Ensure compliance with all relevant regulations and standards of care to maintain high-quality service delivery.

May encounter risk when working with persons with serious mental illness (SMI), including crises, intoxication or impairment, or safety concerns; staff will call local emergency personnel to help de-escalate crises, use crisis intervention techniques, and consult with a supervisor as soon as possible.

Other duties as assigned.

Qualifications

High School Diploma (or equivalent) and Driver's License: Class C.

One year of experience working with one of the following target populations: homeless, severely mentally ill or substance use disorder diagnosed, and/or criminal justice involvement.

Knowledge of behavioral health, criminal justice system, and housing/homelessness, including challenges, needs, resources, interventions, and related content knowledge.

Ability to work occasional evenings and weekends, in inclement weather, and in unsanitary conditions as needed.

Ability to prepare and conduct care management assessments and plans; communicate effectively with groups and individuals both verbally and in writing; instruct and assist in obtaining services and information.

Develop program materials and establish and maintain working relationships with a variety of agencies, organizations, and individuals.

Empathy, compassion, strong communication skills, collaborative nature, resilience, and patience; commitment to whole person wellness.

Bachelor's degree in Healthcare Management, Nursing, or a related field.

Proficiency in motivational interviewing, trauma-informed care, and harm-reduction approaches; knowledge of regulations and standards in residential care.

Bilingual English/Spanish preferred. CNA or MA experience desired. Lived experience is highly desired.
