Resident Care Manager (Healthcare) The RCM implements the established plan of care and delegates tasks in accordance with scope of practice, collaborating with the physician, DON, clinical team members, social services, physical therapy, the resident, and their family. They tailor care to meet individual resident needs.
Responsibilities include:
Monitoring work assignments, providing coaching, training, and feedback to clinical staff including LPNs and CNAs.
Assisting with new admissions, transfers, discharges, and overseeing ongoing resident care to ensure documentation is current, signed, and compliant with policies.
Gathering information and assisting with completion of MDS, observations, and scheduled forms for each resident.
Contributing to resident care conferences and evaluating the interdisciplinary plan of care under supervision of RN or DON.
Monitoring residents' condition and care requirements, taking appropriate actions to meet care needs.
Reviewing nursing care plans to ensure they meet individual needs and physician recommendations.
Reporting changes in resident condition requiring nursing intervention, notifying physicians, DON, and families as appropriate.
Documenting all pertinent information accurately and timely in interdisciplinary notes and reports.
Transcribing and executing or delegating physician orders per Nurse Practice Act.
Assisting in investigating resident incidents and abuse allegations, ensuring timely reporting and follow-up.
Collaborating with DON and RCM RN to maintain compliance with standards and regulations, and participating in quality audits.
Participating in leadership meetings, decision-making, grievance follow-up, and building company initiatives.
Engaging in on-call rotations, including weekends and covering shifts as needed.
Attending continuing education, trainings, and staff meetings to maintain CEUs and professional development.
Performing other duties as assigned.
