Job Summary The Resident Care Coordinator is an experienced Health Care Provider responsible for ensuring that all residents have current care plans and MDS documentation, compliant with Federal, State, and local standards, guidelines, and regulations, as well as facility policies and directives from the Administrator.
Qualifications Current, unencumbered RN or LPN license in this state.
Current CPR certification or certification within 90 days of employment.
Knowledge of OBRA, IDPH, IDPA (Illinois), and Wisconsin DQA standards and documentation methods.
Proficiency in English communication.
Ability to tactfully interact with residents, families, staff, and external agencies.
Leadership skills and ability to work harmoniously with personnel.
Physical Requirements Ability to move intermittently throughout the workday.
Capacity to handle mental and emotional stress.
Adequate senses or prosthetics to meet position demands.
Ability to work with ill, disabled, elderly, and sometimes hostile individuals.
Exposure to infectious diseases, including HIV/AIDS and Hepatitis B.
Computer skills.
Essential Functions Review and recommend revisions to policies and procedures annually.
Oversee scheduling and accuracy of MDS item sets.
Develop and coordinate resident care plans, including multidisciplinary approaches and measurable goals.
Chair care plan conferences and communicate concerns to the Administrator.
Engage residents and families in care planning per RAI guidelines.
Conduct and participate in in-service education related to the RAI process.
Assist in orienting new staff to the RAI process and data entry.
Monitor and ensure consistency in medical records and documentation.
Maintain ICD-9 codes for diagnoses.
Report IT issues promptly.
Participate in QAPI program development and implementation.
Attend meetings as directed.
Perform other duties as assigned.
