Overview We are hiring a Resident Care Coordinator Assistant (RCCA) to join our dynamic care team at Wadsworth Glen Health Care and Rehabilitation Center in Middletown!
7:30a-3:30p schedule with weekly pay.

Responsibilities
Assist the Resident Care Coordinator in the day-to-day activities of the Resident Assessment Process.
Monitor ADL Flow Sheets and observe ADLs to ensure accuracy of coding.
Provide education to CNAs on ADL coding.
Maintain tracking forms to evaluate progress of CNAs needing on-going education and report to the RCC/MDS Director for follow-up.
Enter demographic information into Minimum Data Set (MDS) software.
Complete discharge and re-entry MDS forms.
Schedule resident care plan meetings and send invitations to family members; coordinate teleconference meetings with family members if requested.
Schedule OBRA MDS for all LTC residents; follow up with interdisciplinary team members on non-completed sections of MDS.
Retrieve, print, and fax medical records to managed care companies for utilization review; transmit MDSs to the State database.
Print and file MDS, CAAs, and care plans; retrieve QM reports and email to facility and Athena staff.
Reproduce care plans edited by interdisciplinary team members; track and schedule all Medicare certifications and recertifications; mail and track all Medicare denial letters.
Retrieve/print and fax medical records to QIO for requested appeals; assist with compiling medical records for submission and subsequent level appeals to QIC, ALJ, etc.
Qualifications
Must possess an active and unencumbered CNA license in Connecticut.
Knowledge of medical terminology, filing, typing, computer, and organizational skills.
Previous experience with the MDS process is preferred.
A high school diploma or equivalent.
Must be able to read, write, speak, and understand the English language.
Must possess the ability to make independent decisions when warranted and deal tactfully with personnel, residents, family members, visitors, and the general public.
Must possess the willingness to seek out new methods and principles and incorporate them into existing practices.
