Job Title

This position is primarily responsible for providing general nursing care to patients and will strive to provide these services in a compassionate, professional, ethical, and knowledgeable fashion.

Essential Responsibilities

Under the general direction of the Clinic Director, the Registered Nurse performs a variety of complex, professional, analytical, and confidential medical services. Duties include:

  • Perform all nursing duties in accordance with the state Nursing Act specific to the state you are working in, while adhering to all facility policies and procedures.
  • Perform routine nursing care to assigned patients, while utilizing age specific and universal precaution practices at all times.
  • Record medical histories and symptoms
  • Participate actively in the team-based care model
  • Administers medications, immunizations, performs medication reconciliation. Titrate medications/manages refills per clinic guidelines Takes temperature, pulse, blood pressure, and other vital signs to detect deviations from normal and assess condition of patient.
  • Clinic skills include but are not limited to: vital signs, provides wound care, point of care testing, administration of immunization, and phlebotomy
  • Must be able to perform various sensory requirements such as; vision, speech, smell, touch, manual dexterity, fine motor skills, and hearing in order to assess and treat the patient accurately.
  • Observes patient, records significant conditions and reactions, and notifies supervisor or Physician of patient's condition and reaction to drugs, treatments, and significant incidents.
  • Initiates a patient education plan according to the individualized needs of the patient, as prescribed by Physician and/or hospital policy including patient and family instruction.
  • Effectively utilizes population health methodologies such as patient registry, inreach, outreach, care protocols and team-based care to improve patient and population outcomes
  • Prepares rooms, sterile instruments, equipment, and supplies for surgery.
  • Must be able to work at all applicable locations.
  • Availability to work all operational hours as needed.
  • Other tasks and projects as assigned by supervisor, manager and/or director.
  • Act as a clinical extension of the provider by:
    • Conducting pre-visit chart prep
    • Gathering histories and vitals
    • Assisting in procedures
    • Communicating provider care plans to patients
  • Provide nursing interventions per standing orders and provider direction (e.g., medication administration, wound care, immunizations).
  • Monitor labs, diagnostic results, and medication refills, and follow up with patients as directed by the provider.

Secondary Responsibilities

Attends workshops, training, and meetings as needed, and as requested.

Performs other duties as assigned by supervisor.

Assume general administration duties including fax, filing, copying, etc.

Attend webinars, workshops, training and/or meetings as requested by the Clinic Director.

Perform special project research and data collection activities as requested by the Clinic Director; analyze and interpret data, and prepare reports.

Coordinate special staff events, prepares, and distributes promotional flyers for such events.

Qualifications

Educational Qualifications

1. Bachelor of Science in Nursing (BSN) Required

o Must have completed a Bachelor of Science in Nursing (BSN) from an accredited nursing program.

2. Active State RN (Registered Nurse) License Required

o Must hold a valid RN license in from the state of California Board of Registered Nursing for CA.

Preferred or Additional Qualifications

Current BLS (Basic Life Support) certification required.

ACLS (Advanced Cardiac Life Support) preferred

Experience in primary care, community health, or urgent care preferred.

Experience Requirements

12 years of clinical nursing experience (preferably in ambulatory care, urgent care, or family practice).

Familiarity with electronic health records (EHR) and clinical documentation.

Strong critical thinking and patient assessment skills.

Computer Skills

Knowledge of EPIC, excel, Microsoft Power BI and Tableau is preferred.

This position utilized a computer keyboard, computer monitor, and telephone for prolonged periods.

Specific vision and hearing abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.
