Registered Nurse, RN- Hospice Case Manager, Full Time
Join BAYADA Home Health Care as a full‑time Registered Nurse (RN) Hospice Case Manager at our Brattleboro, VT Hospice office. This role involves providing home visits to hospice clients across Windham County, VT and Cheshire and Sullivan counties in NH.

Responsibilities

Make home visits to clients in designated geographic territories.

Administer medication, wound care, treatments, and procedures; provide comfort care.

Monitor clients' conditions and report changes to the Clinical or Client Services Manager.

Execute and properly document doctors' orders.

Perform client assessments as necessary.

Occasional on‑call after hours and/or weekends.

Qualifications

Current RN license in VT or NH.

Minimum one year of recent, verifiable clinical (medical/surgical) experience.

Prior home care and/or hospice experience preferred.

Solid computer skills; EMR experience preferred.

Ability to travel to cases as assigned.

Compensation
Base Pay: $42–$46 per hour, depending on qualifications.
