Our Case Management team is a dedicated group of nurses and healthcare professionals committed to guiding patients and their families through every step of their hospital journey. We work collaboratively across disciplines to ensure safe, timely discharges, coordinated care, and access to essential resources.

How you'll contribute:

• Coordinates patient care to ensure continuity, optimal outcomes, satisfaction, and cost-effective treatment.
• Collaborates with patients, families, and the healthcare team to support effective discharge planning and resource referrals.
• Addresses concerns from patients, families, and payors while monitoring satisfaction and care quality.

What we're looking for:

• Applicants should have a current Virginia state or compact state RN license.
• Basic Life Support certification is required within 7 days of hire.
• Bachelor's degree in nursing (BSN) preferred.
• 3 years of nursing experience required.
• Experience in Utilization Review, Case Management, Discharge Planning, and Clinical Leadership is highly preferred.
