Registered Nurse (RN) Case Manager, Hospice at Alternate Solutions Health Network. Join to apply for this role.

Responsibilities

  • Symptom management: conduct thorough assessments and work with the patients attending physician and the hospice Medical Director to ensure symptoms are well managed.
  • Plan of Care: evaluate the patients medical condition against the plan of care, individualize the plan, and adjust as needs change.
  • Continuity of Care: manage interdisciplinary care while promoting continuity of care with appropriate admissions, transfers, and discharges.
  • Family Educator/Advocate: counsel patient and family on disease progression and incorporate patient and family in developing the plan of care; listen to patient and family members.
  • Documentation: complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
  • Operations: oversee and supervise total care provided by nurse aides and LPNs; attend in-service training and mandatory agency meetings.

Qualifications

  • Registered Nurse with a current license in the state of employment.
  • Minimum one-year experience as an RN in an acute care setting is preferred.
  • Hospice experience is desired.
  • Valid drivers license and auto insurance in your name as a driver.
  • Capable of all physical demands.

Schedule & Territory

Schedule: Monday - Friday, 9:00am - 5:00pm; 1 weekend per month.

Territory: Trenton, Dearborn, Canton, Detroit.
