Overview
Hiring Case Managers in Colorado. We provide in-home care to former Nuclear Weapons Workers who are suffering from chronic and terminal illnesses, as a result of their previous work environment. Our RN-Case Managers conduct in-home nursing visits for our clients, with visits typically lasting 1-2 hours weekly.
Responsibilities
Conduct in-home nursing visits, perform physical assessments (including vitals), and ensure home safety with medical equipment and supplies.
Fill weekly medication planners and oversee skilled and non-skilled caregivers providing care in the client’s home.
Plan, implement, evaluate care, and participate in client care conferences as needed.
Serve as a local on-site clinical resource and collaborate with regional leadership to identify and improve services.
Assist staffing coordinators with appropriate staffing and scheduling within scope of practice and client needs.
Adhere to nursing delegation guidelines and agency policies.
Participate in interviewing, selection, and ongoing evaluation of clinical personnel as requested.
Provide personnel training, education, and competency validation; review clinical documentation for accuracy and completeness.
Participate in performance improvement initiatives, including medical record reviews and data collection on infections, complaints, and other activities.
Perform supervisory visits as needed to facilitate problem resolution and ensure home chart completeness and timely staffing.
Assist with marketing activities and act as a resource during operating hours as required.
Qualifications
Graduate of a state-approved school of professional registered nursing.
BSN preferred.
Current, unrestricted RN license in the state of practice.
Minimum of two (2) years of nursing experience, including at least one (1) year in home care or a closely related field.
One (1) year of supervisory and/or case management experience preferred.
Current CPR certification.

Standard Rate: $42.00 Hourly plus shift differentials where applicable
Compensation and Schedule
Standard rate: $42.00 hourly, including 100% of hourly wage paid for drive time. Shift differentials may apply.
